HERO House Application
Attendance at HERO House is totally voluntary and is based around your individual needs. The only requirement for Clubhouse membership is that you have a history of mental illness and that you are willing to abide by the one rule of HERO House: To agree not to interfere with the rehabilitation and recovery of other members.
To begin your membership process, you will be asked to complete an application. A member or facilitator will work with you to complete a membership application and some initial referral paperwork. This provides us with some of the personal details that we are required to collect and it gives us some insight into how we can best assist you in your rehabilitation process. This information will remain confidential and will not be shared with outside agencies without your prior consent. You will have the opportunity to take part in the writing of any documents that pertain to you and your rehabilitation.
As part of your new member orientation, you will be asked to attend orientation, which is held every Thursday 11a. to 3 pm. This will give you the opportunity to learn more about the programs we have to offer and some time to work in each of the work units, speak to members and staff to see how the Clubhouse Model works, review the member handbook; and attend a social program activity. If you decide to join our clubhouse, we welcome you as a new member.
